Mae gwasanaethau CONNECT a Blue Army Delta Wellbeing yn gwella gofal cartref, yn lleihau\u2019r nifer o bobl sy\u2019n cael eu haildderbyn i\u2019r ysbyty, ac yn meithrin arloesedd drwy gydweithio ag aelod o\u2019n Rhwydwaith, y Ganolfan Dechnoleg Gofal Iechyd<\/a> (HTC).<\/strong><\/p>\n\n\n\n

Gyda dros 11 miliwn o bobl 65 oed a h\u0177n yng Nghymru a Lloegr yn 2021, a chynnydd rhagamcanol o 16% yng Nghymru erbyn 2030, mae\u2019r galw am iechyd a gofal cymdeithasol yn parhau i godi. Yn draddodiadol, mae trefniadaeth a darpariaeth gofal ar gyfer oedolion h\u0177n bregus wedi wynebu oedi sylweddol. Mae Llesiant Delta<\/a> yn arloesi ym maes darparu gofal cartref ar draws De-orllewin Cymru, gan gynnig gwasanaethau wedi\u2019u teilwra sy\u2019n sicrhau cymorth cynhwysfawr i boblogaethau agored i niwed drwy ei wasanaethau CONNECT a Blue Army.<\/p>\n\n\n\n

Mewn cydweithrediad \u00e2 HTC, nod Llesiant Delta oedd gwerthuso ei wasanaethau, gyda ffocws penodol ar fenter Ymateb i Gwympiadau CONNECT a gwasanaeth Blue Army. Mae'r mentrau hyn yn hwyluso rhyddhau cynnar o'r ysbyty, yn atal aildderbyniadau, ac yn darparu gofal cadarn yn y gymuned. Ceisiodd y gwerthusiad asesu effeithiolrwydd gwasanaethau, nodi rhwystrau i arloesi, a chynnig argymhellion y gellir eu gweithredu ar gyfer gwelliant a graddfa. Drwy ymgysylltu \u00e2 th\u00eem ymchwil Prifysgol Abertawe, nod Llesiant Delta oedd gwneud y gorau o strategaethau lleoli\u2019r gweithlu, symleiddio gwasanaethau, a chryfhau ei safle o fewn y sector gofal iechyd.<\/p>\n\n\n\n

Dechreuodd y prosiect gyda HTC a Llesiant Delta yn sicrhau grant Cyflymu Trosglwyddo Gwybodaeth i Arloesi (AKT2I). Roedd y cam cychwynnol hwn yn cynnwys datblygu cynllun prosiect strwythuredig ac amserlen. Yn dilyn y cais llwyddiannus am grant, cyflwynodd HTC gais moeseg i sicrhau cydymffurfiaeth a pharodrwydd ar gyfer y broses werthuso.<\/p>\n\n\n\n

Cynhaliodd t\u00eem ymchwil HTC asesiad amlochrog o fodelau gofal Llesiant Delta. Cynhaliwyd dadansoddiad annibynnol o ddata meintiol eilaidd, gan gynnwys amseroedd Hefyd, cymerodd 17 o randdeiliaid allweddol\u2014gan gynnwys gweithwyr gofal iechyd proffesiynol o Fwrdd Iechyd Prifysgol Hywel Dda<\/a>, gweithwyr gofal cymdeithasol, a staff Llesiant Delta\u2014ran mewn cyfweliadau lled-strwythuredig. Roedd y cyfweliadau hyn, a gynhaliwyd yn bersonol ac yn rhithwir, yn rhoi cipolwg gwerthfawr ar lwyddiannau a heriau gwasanaethau CONNECT a Blue Army. Amlygodd dadansoddiad thematig o'r trafodaethau hyn heriau allweddol o fewn y sector, megis pontio bylchau gwybodaeth rhwng darparwyr gofal iechyd a thimau gofal cymdeithasol, hwyluso rhyddhau o'r ysbyty, a lleihau aildderbyniadau i'r ysbyty.<\/p>\n\n\n\n

Cynhyrchwyd adroddiad cynhwysfawr i'w rannu \u00e2 Llesiant Delta, yn manylu ar ganfyddiadau allweddol a chanfyddiadau rhanddeiliaid ynghylch llwyddiannau eu mentrau, ynghyd \u00e2 chyfleoedd i fireinio ac ehangu. Caniataodd y gwerthusiad i Brifysgol Abertawe ddatblygu fframwaith wedi'i yrru gan ddata y gall Llesiant Delta ei ddefnyddio i fireinio ei gwasanaethau, gwella'r dyraniad adnoddau, a gosod ei hun ar gyfer ehangu ledled Cymru yn y dyfodol.<\/p>\n\n\n\n

Tynnodd Sarah Vaughan o Delta Wellbeing<\/strong> sylw at effaith y cydweithio hwn, gan nodi, \u201cMae\u2019r cydweithio rhwng Prifysgol Abertawe a Llesiant Delta wedi bod yn gyfle cadarnhaol, gan feithrin trosglwyddo gwybodaeth yn llwyddiannus. Mae'r prosiect hwn wedi annog dysgu pellach ac wedi amlygu'r angen am gydweithio yn y dyfodol. Mae wedi dangos effeithiolrwydd gwaith t\u00eem ac ymrwymiad ar y cyd i gyflawni nodau cyffredin.\u201d<\/p>\n\n\n\n

Mae'r cydweithio hwn yn enghraifft o sut gall partneriaethau'r Rhwydwaith ysgogi arloesedd, gwella'r modd y darperir gofal iechyd, a sicrhau bod poblogaethau sy'n agored i niwed yn cael y cymorth ansawdd uchel sydd ei angen arnynt.<\/p>\n\n\n\n
